Creating and Administering Findings and Actions

Audit User (AUDITOR) or higher permissions are required.
This article walks you through the process of adding Findings to Questions and Actions to Findings.
If Question Metadata Values have associated Finding Types, the system will trigger that a Finding is automatically raised when selecting that value as your Question Answer. Additionally, if there is a Workflow associated with the Finding Type, this process will also be triggered.

1. Creating a Finding with Question
To create a Finding with the Question whilst recording the Answer:
- Select the Click to Raise a Finding icon in the Answer area of the Question.
- Enter a Finding Title.
- Select a Finding Type from the dropdown.
- Change the Finding Owner (if different from yourself).
- Click Save.
Optionally, you can add associations of Standards and/or Organisational Areas to the Finding.
To create associations:
- Select the relevant Associate... icon at the top of the screen.
An overlaid window will be displayed, similar to this example for Organisational Areas Selection.
- Filter the list using the search criteria at the top of the form.
- Select the required associations by ticking the boxes in their rows.
- Click the Associate... icon.
The selected entities for association will appear in the Associated Items list.
2. Adding Actions
To add an Action to the Finding:
- Click the Add New Action icon.
The Add New Action overlaid window will be displayed.
- Click Yes to raise an Action.
- Select an Action Type from the dropdown.
Title and Description will be drawn from the chosen Action Type template but can be altered. - Change the Action Owner (if different to yourself).
- Set a Target Date.
This is a date by which the Action should be completed. - Complete the remaining optional fields.
- Click Save.
You will be asked if you would like to raise another action.
Clicking Yes will restart the above process, while clicking No will return you to the Action Details screen.
3. Issuing Actions to Owners
To issue the Action, click the Issue button to the right of the title bar.
This will update the Action Details screen and the Action will appear in the Owner's To Do List. They will be notified by email.
4. Viewing Findings and Actions
To view Findings associated with the Audit:
- Open the Audit containing the Findings you want to review.
- Switch to the "Findings" tab.
- You can view associated Actions by selecting the Reveal icon to the left of the Finding title.
The screen will expand to display the information.
- If required, you can amend the Finding details and add additional Associated Items from this view.
You can also amend the associated Actions, re-sequence, create or even delete an Action prior to Issuing the first Action(s).
